目的:评价乳腺癌胸部相对不常见转移的CT表现,提高对本病的认识。方法:回顾性分析经手术或穿刺病理组织学确诊72例乳腺癌胸部相对不常见转移患者的CT影像资料。结果:胸壁囊性转移1例,CT表现呈囊性肿块,有薄壁及附壁结节;胸壁转移伴自发性气胸5例,CT表现气胸附近跨胸壁破坏性肿块;肺内磨玻璃样及晕样转移转移瘤27例中12例CT表现呈片状磨玻璃密度影,15例晕样转移中央区域呈软组织密度,周缘部呈磨玻璃样密度,其中3例伴晕中小空泡状气体影;肺内空洞样转移28例中厚壁空洞19例,薄壁空洞14例,空泡样空洞3例,午间猫眼样空洞2例,大囊袋样空洞2例,球核样空洞2例,13例同时具有≥2种空洞性表现;单发肺内转移中1例CT表现为含壁结节的单发空洞病灶,1例为含毛刺及分叶的单发软组织结节;钙化性肺转移7例CT表现以斑片及斑点状钙化为主;心包膈淋巴结转移2例CT表现为心膈区3~4枚椭圆形软组织结节。结论:认识乳腺癌胸部不常见转移的CT表现,对乳腺癌诊断、鉴别诊断及治疗具有重要意义。
OBJECTIVE: To evaluate the CT findings of relatively uncommon breast cancer metastases and to raise awareness of the disease. Methods: CT images of 72 cases of breast cancer with relatively uncommon metastasis from breast cancer confirmed by operation or biopsy were retrospectively analyzed. Results: One case of cystic metastasis of the chest wall showed cystic mass with thin wall and mural nodules. Five cases had chest wall metastasis accompanied by spontaneous pneumothorax. CT showed the destructive thoracic wall mass near the pneumothorax. Twelve cases of halo-metastatic metastases showed flake-shaped glass densities in 12 cases, soft-tissue densities in 15 cases of central haloid-like metastases, and glass-like densities in the periphery, of which 3 cases were associated with small, . There were 19 cases of middle-thick wall empty hole transfer in 14 cases of lung cavity, 14 cases of thin-wall hollow, 3 cases of vacuole-like cavity, 2 cases of cat-like cavity in midday, 2 cases of large pocket-like cavity, Thirteen patients had at least two types of cavities. One case with intrapulmonary metastasis showed single lesions with nodules and one with soft tissue nodules with burr and lobes. Calcified lung The CT findings of 7 cases were mainly patchy and speckled calcification. The CT findings of 2 cases of pericardium diaphragmatic lymph node metastasis were 3 ~ 4 oval soft tissue nodules in the diaphragmatic area. Conclusion: It is of great significance to recognize the CT findings of breast cancer with uncommon metastasis and to diagnose, differentiate and treat breast cancer.
