目的:探讨高频彩色多普勒超声检测乳腺癌供血动脉的胸外侧动脉、胸廓内动脉的血流动力学改变及胸壁厚度测定,对乳腺癌改良根治术后辅助放疗的参考价值。方法:采用彩色多普勒超声测量广东籍贯30名正常女性及30例乳腺癌患者的胸外侧动脉、胸廓内动脉血流测定参数,及乳腺癌改良根治术后辅助放疗前及放疗中照射剂量分别为10、30和50Gy时术侧胸壁厚度,对所测量数据进行统计分析。结果:乳腺癌组患侧胸外侧动脉与正常对照组、乳腺癌健侧胸外侧动脉的EDV、RI比较,差异均有统计学意义,P<0.01。乳腺癌组患侧胸廓内动脉与正常对照组、乳腺癌健侧胸廓内动脉的SPV、RI比较,差异无统计学意义,P>0.05;内径、EDV相比较差异有统计学意义,P<0.01。乳腺癌改良根治术后辅助放疗患侧胸壁厚度在治疗过程中的变化,差异有统计学意义,P<0.01;术侧平均胸壁厚度(1.314 9±0.337 67)cm。结论:彩色多普勒超声检测胸外侧动脉的血流参数比胸廓内动脉的血流参数对乳腺癌更有临床意义,测定术侧胸壁厚度为优化电子线照射的放射治疗计划提供重要的测量数据。
Objective: To investigate the value of high frequency color Doppler ultrasonography in the detection of hemodynamics and thoracic wall in the thoracic artery and thoracic artery of breast feeding artery, and to evaluate the value of adjuvant radiotherapy after radical mastectomy for breast cancer. Methods: Thoracic artery, thoracic artery blood flow measurement parameters in 30 normal women and 30 breast cancer patients from Guangdong native place were measured by color Doppler echocardiography, and the dose of radiotherapy before and after adjuvant radiotherapy At 10, 30, and 50 Gy, the thickness of the operative side chest wall was statistically analyzed for the measured data. Results: There were significant differences in EDV and RI between the contralateral thoracic artery in breast cancer group and the normal control group and in the contralateral thoracic artery of breast cancer (P <0.01). There was no significant difference in SPV and RI between contralateral internal thoracic artery in breast cancer group and normal control group, and breast contralateral thoracic artery, P> 0.05; there was significant difference between internal diameter and EDV, P <0.01 . There was significant difference in the thickness of thoracic wall after adjuvant radiotherapy for modified radical mastectomy (P <0.01). The mean thoracic wall thickness (1.314 9 ± 0.337 67) cm in the operative side was statistically significant. CONCLUSIONS: Color Doppler ultrasonography is more clinically useful for determining the blood flow parameters of the extrathoracic artery than the blood flow parameters of the thoracic artery and provides important measurement data for radiotherapy planning to optimize electron beam irradiation .
