目的探讨口腔鳞状细胞癌组织及配对正常口腔黏膜组织中基质金属蛋白酶1(MMP1)、基质金属蛋白酶抑制剂1(TIMP1)及基质金属蛋白酶抑制剂3(TIMP3)蛋白和mRNA表达及临床意义。方法研究对象为2005年6月~2008年11月在河北医科大学第四医院口腔科首诊的51例口腔鳞状细胞癌患者。应用免疫组化及RT-PCR法检测癌组织和配对正常口腔黏膜组织MMP1、TIMP1和TIMP3的蛋白和mRNA表达。结果在口腔鳞状细胞癌组织和癌旁正常口腔黏膜组织中MMP1蛋白的阳性表达率分别为82.4%(42/51)和17.6%(9/51),两者比较有统计学意义(P<0.05),mRNA的相对表达量为高于正常口腔黏膜组织的相对表达量(P<0.05);TIMP1蛋白的阳性表达率分别为72.5%(37/51)和94.1%(48/51),差异有统计学意义(P<0.05),mRNA的相对表达量差异有统计学意义(P<0.05);TIMP3蛋白的阳性表达率分别为78.4%(40/51)和100%(51/51),差异有统计学意义(P<0.05),mRNA的相对表达量差异有统计学意义(P<0.05)。在肿瘤组织中MMP1/TIMP1/TIMP3蛋白的表达率和mRNA的相对表达量与组织病理学分级﹑有无淋巴结转移差异有统计学意义(P<0.05),而与患者的年龄无关(P>0.05)。结论 MMP1的高表达与TIMP1、TIMP3低表达与口腔鳞状细胞癌的发生、组织病理学分级﹑淋巴结转移有关,可用于判断预后及评估转移风险。
Objective To investigate the expression of MMP1, TIMP1 and TIMP3 in oral squamous cell carcinoma (OSCC) and their matched normal oral mucosa tissues and their clinical significance. Methods The object of study was from January 2005 to November 2008 in 51 cases of oral squamous cell carcinoma in the Department of Stomatology, the Fourth Hospital of Hebei Medical University. Immunohistochemistry and RT-PCR were used to detect the protein and mRNA expressions of MMP1, TIMP1 and TIMP3 in normal tissues and matched normal oral mucosa tissues. Results The positive rates of MMP1 protein in oral squamous cell carcinoma tissues and adjacent normal mucosa tissues were 82.4% (42/51) and 17.6% (9/51), respectively, with statistical significance (P < (P <0.05). The positive expression rates of TIMP1 protein were 72.5% (37/51) and 94.1% (48/51) respectively, the difference was significant (P <0.05). There was significant difference in the relative expression of mRNA (P <0.05). The positive rates of TIMP3 protein were 78.4% (40/51) and 100% (51/51) The difference was statistically significant (P <0.05), the relative expression of mRNA difference was statistically significant (P <0.05). The expression of MMP1 / TIMP1 / TIMP3 and the relative expression of MMP-1 / TIMP1 / TIMP3 in tumor tissue were significantly different from histopathological grade and lymph node metastasis (P <0.05), but not with age ). Conclusion The high expression of MMP1, the low expression of TIMP1, TIMP3 and the occurrence of oral squamous cell carcinoma, histopathological grade and lymph node metastasis may be used to judge the prognosis and evaluate the risk of metastasis.
