牙科手术中一个明显可见的现象是,无局部麻醉的条件下,任何在牙本质表面的牙科操作都会导致疼痛。热、冷、渗透压、电刺激、气流、各种药物等刺激作用于牙本质表面时,均可引起疼痛,称为牙本质疼痛。大量组织化学、放射自显影和电镜研究结果生动的描绘了造牙本质细胞与神经纤维的关系。但经牙本质到神经末稍的疼痛传导机理还无定论。因此,对于解释某些临床现象和防治牙病还没有有效的办法。处在这样一个认识的发展阶段,提出某些假说将有助于疼痛传导机制的研究。本文试对疼痛传导机理研究中假说的建立的辨证法作一探讨。
One of the obvious signs of dental surgery is that any dental procedure on the dentine surface can cause pain without local anesthesia. Heat, cold, osmotic pressure, electrical stimulation, airflow, a variety of drugs and other stimuli on the dentin surface, can cause pain, known as dentin pain. A large number of histochemical, autoradiography and electron microscopy results vividly depicts the relationship between dentin cells and nerve fibers. However, the mechanism of pain transmission through the dentine to the nerve endings is inconclusive. Therefore, there is no effective way to explain some clinical phenomena and prevent dental diseases. At this stage of development of understanding, some hypotheses are proposed that will contribute to the study of pain transmission mechanisms. This paper attempts to discuss the hypothesis established in the study of the mechanism of pain transmission.
