目的调查掌握2008-2010年广东省3市常住人口格林-巴利综合征(Guillain-Barre syndrome,GBS)基础发病率,分析其流行病学特征及发病原因,为GBS进一步研究提供参考依据。方法收集2008-2010年广东省3市人口学基本信息和预防接种数据,通过医院信息管理系统(HIS)和手工查询并由专家组核实GBS病例个案资料,采用描述流行病学方法分析病例发生的三间分布、病因,估算常住人口基础发病率。结果 2008-2010年共调查医院183间,搜索住院病例4 928 425例,GBS确诊病例602例,其中常住人口231例,全年各月均有病例报告,第1、2季度较高。常住人口GBS估算年均发病率为3.41/100万,其中2008年最高(4.02/100万),年龄别估算发病率以0~4岁组儿童最高(5.77/100万),性别估算发病率男性(4.36/100万)高于女性(2.38/100万),职业以离退人员和散居儿童为主。发病前2月内有疫苗接种史者3例(0.11/100万剂次),分别为流脑疫苗(0.41/100万)、脊灰疫苗(0.30/100万剂次)和麻疹疫苗(0.26/100万剂次)。结论 3市常住人口GBS估算发病率小于文献报道的发生率(0.6/10万~1.9/10万),男性估算发病率高于女性,0~4岁估算发病率最高;有疫苗接种史者GBS估算发病率小于常住人口估算发病率,预防接种不会增加GBS发生的风险。
Objective To investigate the basic incidence of Guillain-Barre syndrome (GBS) in 3 permanent residents in Guangdong Province from 2008 to 2010, and to analyze the epidemiological characteristics and causes of GBS, so as to provide a reference for further study of GBS. Methods The demographic data and vaccination data of three cities in Guangdong Province during 2008-2010 were collected. Case information of GBS cases was checked by HIS and verified by expert group, and the cases were analyzed by descriptive epidemiology Three distribution, etiology, to estimate the basic incidence of resident population. Results A total of 183 hospitals were searched from 2008 to 2010, 4 928 425 were searched for inpatients, and 602 cases were diagnosed by GBS. Among them, 231 cases were resident population. Cases were reported throughout the year and higher in the first and second quarters. The estimated annual average prevalence of resident population GBS is 3.41 / 1 million, of which the highest in 2008 (4.02 / 1 million), the age-estimated incidence of children aged 0 to 4 years (5.77 / 100 million), the incidence of male gender estimates (4.36 / 1 million) higher than females (2.38 / 1 million), occupations were mainly retired staff and scattered children. There were 3 cases (0.11 / 1 million doses) of vaccination history within 2 months before onset, which were meningitis vaccine (0.41 / 1 million), polio vaccine (0.30 / 1 million doses) and measles vaccine (0.26 / 100 million doses). Conclusions The incidence of GBS estimated by resident population in 3 cities is less than that reported in the literature (0.6 / 100000 ~ 1.9 / 100000). The estimated incidence of males is higher than that of females, and the highest incidence is estimated from 0 to 4 years old. The estimated incidence is less than that of the resident population, and vaccination does not increase the risk of developing GBS.
