目的:研究南方某部反流性食管炎(RE)的患病情况及其与幽门螺杆菌(Hp)的关系。方法:我们的前期问卷调查研究结果显示GERD军人102例,根据入选标准有91例GERD患者入选,其中低分组(12~15分)47例、中分组(16~20分)34例、高分组(>20分)10例。另随机抽取60例非GERD军人为对照组。共151例军人接受胃镜检查,同时采用14C-尿素呼气试验与取胃窦小弯粘膜行组织快速尿素酶试验两种方法检测Hp,两种方法结果均阳性者判为Hp阳性。结果:GERD组与对照组RE检出率分别为42.86%(39/91)、3.33%(2/60),差异显著(P<0.05);但GERD各积分组之间RE检出率无显著差异(P>0.05)。GERD组中39例RE,其中A、B级分别22例、13例(共占89.74%),C、D级分别3例、1例(共占10.26%);另检出8例BE(8/91,8.79%),均为岛型、舌型或混合型。症状积分与RE分级情况线性回归R值为0.276(P>0.05),症状积分与RE级别之间无明显线性关系。RE组Hp感染率为58.97%(23/39),略低于对照组(P>0.05)。结论:军人RE检出率及Hp感染率均不低,RE检出率与Hp感染无明显关联。
Objective: To study the prevalence of reflux esophagitis (RE) and its relationship with Helicobacter pylori (Hp) in a southern part of the country. Methods: Our previous questionnaire survey revealed 102 GERD soldiers, 91 of whom were classified as GERD based on the inclusion criteria, including 47 in the low (12 to 15), 34 in the middle (16 to 20), high (> 20 points) 10 cases. Another 60 randomly selected non-GERD soldiers as the control group. A total of 151 military personnel underwent gastroscopy. Hp was also detected by 14C-urea breath test and Tissue Rapid Urease Test in gastric mucosa with small flexion and mucosa. Hp was positive for both methods. Results: The detection rates of RE in GERD group and control group were 42.86% (39/91) and 3.33% (2/60), respectively, with significant difference (P <0.05). However, there was no significant difference in RE detection rate between GERD and control groups Difference (P> 0.05). There were 39 cases of RE in GERD group, including 22 cases of A and B grade, 13 cases (89.74%), 3 cases of C and D grade (1 case, 10.26%), and 8 cases of BE /91,8.79%), are island, tongue or mixed type. There was no significant linear relationship between symptom score and RE level when the linear regression of symptom score and RE grade was 0.276 (P> 0.05). The infection rate of Hp in RE group was 58.97% (23/39), slightly lower than that in control group (P> 0.05). Conclusion: The detection rate of RE and the infection rate of Hp are not low, the detection rate of RE is not significantly associated with Hp infection.
