背景:对于儿童注意缺陷多动障碍(attentiondeficiencyandhyperactivitydisorder,ADHD)的诊断和评估,多年来一直采用主观的量表评定法,如何用客观的检查手段进行更正确、合理的诊断,是近年来探索的一个课题。目的:探讨视听整合连续测试对在ADHD儿童中的评估价值,以及客观、易量化的评估ADHD的方法。设计:横断面研究。地点和对象:对2003-07/10在无锡市儿童医院心理行为门诊初诊的瑞文智商筛查正常,症状持续半年以上,疑似ADHD的全部儿童157例。干预:采用视听整合连续测试进行评估,分析被诊断为ADHD儿童听、视觉注意能力的差异。主要观察指标:视听整合连续测试所得18个商数指标。结果:反应控制商数与注意力商数结果提示:注意力缺陷的发病率高于自控力差、冲动的发病率,差异有显著性意义(χ2=37.74,P<0.01);在反应控制商数中,听觉的控制能力比视觉的控制能力差,差异有显著性意义(χ2=14.68,P<0.01);注意力商数中的听、视觉对比,差异无显著性意义(χ2=3.56,P>0.05)。视听整合连续测试诊断为ADHD的96例儿童听觉的一致性(68例)、警惕性(66例)、注意力商数(50例)<85分值的比例较高;视觉的警惕性(58例)、速度商数(61例)<85分值的比例较高,差异均有显著性意义(χ2=6.75~22.81,P均<0.01)。结论:视听整合连续测试能?
Abstract BACKGROUND: The subjective scale assessment method has been used for the diagnosis and assessment of attention deficit hypehoid disease (ADHD) in children for many years. How to make more correct and reasonable diagnosis by objective examination is one of the most recent studies Question. OBJECTIVE: To explore the value of continuous audiovisual test in the evaluation of children with ADHD, and objective and easy-to-assess methods for assessing ADHD. Design: Cross-sectional study. Location and Subjects: Screening of Ruiwen IQs newly diagnosed at the Children’s Hospital Psychological Behavioral Hospital of Wuxi City from July to July 2003 was normal. The symptoms persisted for more than six months and 157 of all children suspected of having ADHD. Intervention: A continuous assessment of audio-visual integration was conducted to assess the differences in hearing and visual attention ability of children diagnosed with ADHD. MAIN OUTCOME MEASURES: 18 quotient indexes obtained through continuous testing of audiovisual integration. Results: The results of reaction control quotient and attentional quotient indicated that the incidence of attention deficit was higher than that of self-control and impulsivity, the difference was significant (χ2 = 37.74, P <0.01) (Χ2 = 14.68, P <0.01). There was no significant difference in hearing quotient and visual contrast in attention quotient (χ2 = 3.56, P> 0.05). Auditory consistency (n = 68), vigilance (n = 66), attentional quotient (n = 50) <85; auditory awareness (Χ2 = 6.75-22.81, P <0.01).
