首次应用小剂量心得安(口服10mg)后,迅速引起严重致命性反应,谓之首剂反应综合征,临床较罕见。现将所遇1例报道如下。患者女,65岁。因咽痛、咳嗽、畏寒和发热4天于1991年1月23日入院。诊断为急性上呼吸道感染,给予静脉滴注青霉素和氨苄青霉素治疗。住院第3天体温恢复正常,但仍有心悸,不气急。心界不大,心律齐,心率120次/分,无病理性杂音,血压16/10kPa。当天晚21∶00时心电图示窦性心动过速。21∶30时给服心得安10mg,1小时55分后出现头晕,面色苍白,口唇紫绀,大汗淋漓,
