目的了解新生儿下呼吸道产超广谱β-内酰胺酶(ESBLs)细菌的感染情况及耐药性。方法应用纸片扩散确证试验检测ESBLs,Kirby-Bauer(K-B)法进行药敏监测。结果肺炎克雷伯菌产酶率为57.9%;大肠埃希菌为47.7%。ESBLs产生株对抗生素的耐药率明显高于ESBLs非产生株;ESBLs产生株对氨苄西林、哌拉西林、奥格门丁及三代头孢菌素高度耐药,除头孢他啶以外,耐药率均在80%以上;交叉耐药现象显著。产ESBLs菌对亚胺培南的的敏感率为100%。结论新生儿下呼吸道感染者产ESBLs情况严重;亚胺培南仍然为最有效的抗生素;应采取措施控制产ESBLs细菌的感染。
Objective To understand the infection and drug resistance of ESBLs-producing bacteria in neonates with lower respiratory tract. Methods ESBLs were tested by disk diffusion test and Kirby-Bauer (K-B) method was used for drug susceptibility testing. Results Klebsiella pneumoniae producing enzyme rate was 57.9%; Escherichia coli was 47.7%. The resistance rates of ESBLs-producing strains to antibiotics were significantly higher than that of non-ESBLs-producing strains. ESBLs-producing strains were highly resistant to ampicillin, piperacillin, octreotide and third generation cephalosporins. More than 80%; cross-resistance is significant. The susceptibility of ESBLs-producing strains to imipenem was 100%. Conclusions ESBLs are very serious in neonates with lower respiratory tract infection. Imipenem is still the most effective antibiotic. Infection with ESBLs-producing bacteria should be controlled.
