目的初步评估多层面螺旋CT血管造影(MSCTA)在颅内动脉瘤诊治中的临床应用价值。方法对57例临床怀疑颅内动脉瘤病例应用Aquilion多层面螺旋CT行MSCTA检查,重建方法包括多层面重建(MPR),遮盖容积重建(SVR),最大密度投影(MIP)以及外视镜(Fly-around)和内镜式血管表面重建(Fly-through)。49例确诊为颅内动脉瘤,均经手术证实。还在术后对5例病例进行了MSCTA复查。结果MSCTA共发现动脉瘤49个,与DSA和手术相比,检出率分别为100%和98%;而且MSCTA能够清楚显示动脉瘤及其与周围结构之间的解剖关系。结论可将MSCTA作为颅内动脉瘤诊断的首选检查手段。在指导动脉瘤治疗方面MSCTA可对DSA起到重要补充作用,并可在动脉瘤夹闭术后复查中发挥作用。
Objective To evaluate the clinical value of multi-slice spiral CT angiography (MSCTA) in the diagnosis and treatment of intracranial aneurysms. Methods Fifty-seven cases of suspected intracranial aneurysms were examined by MSCTA with the aid of Aquilion MSCT. The reconstructions included MPR, SVR, MIP and Fly. -around) and endoscopic vascular reconstruction (Fly-through). 49 cases were diagnosed as intracranial aneurysms, all confirmed by surgery. Five cases were still underwent MSCTA after surgery. Results A total of 49 aneurysms were found in MSCTA. The detection rates were 100% and 98% respectively compared with those of DSA and surgery. Moreover, MSCTA clearly showed the anatomic relationship between the aneurysms and the surrounding structures. Conclusion MSCTA can be used as the first choice for the diagnosis of intracranial aneurysms. MSCTA plays an important role in DSA in guiding the treatment of aneurysms and can play a role in the review of aneurysm after clipping.
