目的:研究各种病源菌性脑膜脑炎、脑膜炎整个病程中血流动力学变化,及其和病情严重程度、预后的关系。方法:对40例病毒性脑膜脑炎(VEM),13例结核性脑膜炎(TBM),7例化脓性脑膜炎(PM)患者和60例正常对照组进行TCD观察。结果:发现TBM69.27%、PM42.8%、VEM28.05%发生血管痉挛。TBM血管痉挛在发病一周左右发生,持续时间长,可达3个月左右。PM、VEM血管痉挛在发病第二天即可发生,持续1周左右。结论:病源菌不同时血流动力学的影响不同,TBM、PM发生血管痉挛者较多,TCD可判断TBM、PM病情严重程度及TBM的预后。
Objective: To study the hemodynamic changes of all kinds of pathogenic meningoencephalitis and meningitis during the whole course of the disease and its relationship with the severity and prognosis. Methods: TCD was performed in 40 patients with viral meningoencephalitis (VEM), 13 with tuberculous meningitis (TBM), 7 with purulent meningitis (PM) and 60 normal controls. Results: It was found that TBM69.27%, PM42.8%, VEM28.05% vasospasm. TBM vasospasm occurs in the incidence of about a week, long duration, up to 3 months. PM, VEM vasospasm in the incidence of the next day can occur for 1 week or so. Conclusion: The pathogenic bacteria do not have different effects on hemodynamics at the same time. There are many vasospasm in TBM and PM. TCD can judge the severity of TBM and PM and the prognosis of TBM.
