研究静注丹参对兔血浆血管活性因子内皮素(ET)、一氧化氮(NO)水平的影响,分析血浆ET/NO比例与皮瓣微循环血量的相关性,以揭示丹参改善皮瓣微循环的机理,皮瓣分成丹参组、生理盐水两组,抽血测定两组皮瓣掀起前后血浆ET、NO水平动态变化,绘出ET/NO比例曲线,并用激光多谱勒血流仪监测皮辨微循环血量。结果:两组血浆ET/NO比例在皮瓣掀起后1h开始明显上升,丹参组3h后下降,1d后恢复到术前水平;生理盐水组血浆ET/NO比例上升较丹参组更显著,且下降缓慢,5d后才接近术前水平,皮瓣掀起后两组皮瓣微循环血量急剧下降,1h后降到最低;生理盐水组皮瓣微循环血量在掀起后各时相下降副度明显大于丹参组(P<0.01)。生理盐水组皮瓣掀起后1h、3h血浆ET/NO比例与皮瓣微循环血量成显著负相关(γ=-0.89,P<0.01)。术后14d观察丹参组皮瓣成活长度较生理盐水组皮瓣明显增加(P<0.01)。提示丹参是通过优化血浆ET/NO比例以改善皮瓣微循环,提高皮瓣成活长度。
To study the effects of intravenous injection of salvia miltiorrhiza on plasma endothelin (ET) and nitric oxide (NO) levels in plasma of rabbits, and to analyze the correlation between plasma ET/NO ratio and microcirculation blood volume of flaps to reveal that Danshen improves flap micro The circulation mechanism was divided into Salvia miltiorrhiza group and normal saline group. Results: The plasma ET/NO ratio in the two groups began to rise at 1 h after the flap lifted. Salvia miltiorrhiza group decreased after 3 h and recovered to the preoperative level after 1 d. The plasma ET/NO ratio in the saline group was more significant than that in the Salvia miltiorrhiza group and decreased. Slowly, the preoperative level was approached 5 days later. After the flap was lifted, the microcirculation of blood volume in the two groups dropped sharply and dropped to a minimum after 1 hour. The blood volume of the microcirculation of the flap in the saline group decreased significantly after each phase was lifted. Greater than Salvia miltiorrhiza group (P<0.01). In the normal saline group, plasma ET/NO ratio was significantly negatively correlated with flap microcirculation blood volume 1 h and 3 h after flap lift (γ=-0.89, P<0.01). At 14 days after surgery, the skin flaps of salvia miltiorrhiza group were significantly longer than normal saline group (P<0.01). It is suggested that Salvia miltiorrhiza is to improve the microcirculation of the flap by optimizing the ratio of plasma ET/NO and increase the survival length of the flap.
