患儿,男,4岁。因误服止咳喘热参片(河南省联合制药厂生产)20粒,1小时后出现烦燥不安,两手抓空舞动,急诊来院。体检:神志恍惚,烦燥不安,面颊潮红,结膜充血,双侧瞳孔散大(约0.6cm),对光反应迟钝,口唇干燥,轻度发绀,呼吸稍急促,肺部清晰,心音有力,心率155次,腹软、稍胀。诊断为止咳喘热参片中毒。给予催吐(吐出五片完整药片)、洗胃、镇静、输液等综合治疗,同时肌肉注射新斯的明0.3mg。历时约12小时,症状逐渐消失而痊愈。
Children, male, 4 years old. Because of cough and asthma due to mistaken heat tablets (produced by the joint pharmaceutical factory in Henan Province) 20, 1 hour after the irritable, empty-handed with both hands, emergency room. Physical examination: trance, irritability, cheek flushing, conjunctival hyperemia, bilateral mydriasis (about 0.6cm), slow reaction to light, dry lips, mild cyanosis, shortness of breath, clear lungs, strong heart sounds, heart rate 155 times, belly soft, slightly swollen. Cough and heat diagnosis tablets poisoning. Give vomiting (spit out five complete tablets), gastric lavage, sedation, infusion and other comprehensive treatment, while intramuscular neostigmine 0.3mg. About 12 hours, the symptoms gradually disappear and heal.
