目的探讨切开复位交叉克氏针内固定治疗儿童陈旧性桡骨颈骨折的疗效。方法自2009-01—2013-02采用切开复位克氏针交叉内固定治疗儿童陈旧性桡骨颈骨折13例,2枚克氏针进针点均位于桡骨头安全区。结果术后13例切口均一期愈合,无感染发生。所有患儿均获得随访1~3年,平均2.4年。骨折复位良好,基本恢复关节平面。X线片显示骨折愈合时间为4~6周,无骨不连、骨折延迟愈合、桡骨小头无菌性坏死及肘部畸形。肘部屈伸活动度改善40°~70°,平均56°。前臂旋转活动均恢复好。结论桡骨颈骨折治疗重点在于早诊断、早治疗,避免发生陈旧性骨折。对于骨折不愈合及畸形愈合的陈旧性骨折,尽早行切开复位交叉克氏针内固定手术可取得较好的治疗效果。
Objective To investigate the curative effect of open reduction and cross Kirschner wire internal fixation on children’s old radial neck fracture. [Methods] Thirteen cases of old radial neck fracture were treated with open reduction and Kirschner wire cross internal fixation from 2009-01-2013-02. Two Kirschner wire needle points were located in the radial head safety zone. Results All the 13 incisions healed after operation and no infection occurred. All children were followed up for 1 to 3 years, an average of 2.4 years. Fracture reduction is good, the basic restoration of joint plane. X-ray film showed fracture healing time of 4 to 6 weeks, nonunion, delayed union of fractures, aseptic necrosis of the radial head and elbow deformity. Elbow flexion and extension activities improved 40 ° ~ 70 °, an average of 56 °. Forearm rotation activities are restored. Conclusion The focus of radial neck fracture treatment is early diagnosis and early treatment to avoid the occurrence of old fractures. For the non-union fracture and malunion of old fractures, as early as possible underwent open reduction and cross Kirschner wire fixation can achieve better therapeutic effect.
