颅内动脉瘤破裂最常见的CT表现是蛛网膜下隙出血，但也存在一些影像学上不以单发蛛网膜下隙出血为首发CT表现的颅内动脉瘤，本文报道1例以颞叶出血并破入脑室为首发CT表现的后交通动脉瘤。对该病例的病史、入院查体、相关影像资料、术中情况及预后给予报道。并通过文献复习归纳分析该类病例的病史及影像特点、诊断要点及术中操作技巧。
