患者女，36岁，农民。因腹泻、便秘交替2个月伴发热3天，于1996年4月8日入院。2个月前患者无明显诱因出现脐周不适，腹泻便秘交替。腹泻时每天排糊状稀便3～4次，不含粘液及脓血，无里急后重，继而2～3天排硬便1次，无明显腹痛、呕吐、便血、发热及盗汗。多次在当地医疗站中药治疗，症状无缓解，头晕、乏力渐加剧。近3天出现发热，体温波动于37．0℃～39．6℃，无四肢关节肿痛，皮疹及皮肤粘膜出血，当地对症治疗无效。发病以来食欲明显减退，体重下降5公斤。患者既往体健，无结核病史。体查：体温37．2℃，脉搏85次／分，呼吸20次／分，血压13／9kPa。贫血貌，消瘦，皮肤粘膜无出血点及皮疹，浅表淋巴结无肿大，巩膜无黄染，咽充血，扁桃体无肿大，齿龈无肿胀渗血，胸骨无压痛，心肺无异常。腹平软，脐左侧可触及约3cm×8cm纵行条索状包块，质地较硬轻压痛，活动度尚可，肝脾肋下未触及，腹水征（－），肠鸣音正常，肛门指诊无异常。实验室及特殊检查：血红蛋白60g／L，白细胞5．2×109／L，中性0．76，淋巴0．23，大单核0．01，血小板200×109／L，网织红细胞0．005，血沉4mm／h，大便常规：红细胞少许，大便隐血（十），尿常规正常，肝肾功能正常，血清白蛋白28g／L，球蛋白24g／L，蛋白电泳γ-球蛋?
