目的 了解人类端粒酶逆转录酶 (hTRT)在脑膜瘤中的表达 ,探讨hTRT与脑膜瘤恶性程度的关系及预后意后。方法 应用免疫组化技术检测 4 3例脑膜瘤中hTRT和增殖细胞核抗原 (PCNA)的表达 ,对脑膜瘤不同病理级别组之间、初发组和复发组之间的hTRT阳性率进行对比分析 ,并比较hTRT阳性组和阴性组之间PCNA的标记指数。结果 4 3例脑膜瘤中 ,hTRT阳性 19例 ,阳性率为 4 4%。其中良性组、非典型组和恶性组的hTRT阳性率分别为 11 5 %、90 %和 10 0 % (P <0 0 1) ;复发组和初发组的hTRT阳性率分别为 75 %和 32 % (P <0 0 1) ;hTRT阳性组和阴性组的PCNA标记指数分别为 (38 4± 13 3) %和 (11 8± 3 3) % (P <0 0 1)。结论 hTRT不仅存在于非典型性、恶性型脑膜瘤中 ,而且还存在于部分形态学上良性的脑膜瘤中 ;hTRT的表达与脑膜瘤的恶性程度有密切关系 ,预示肿瘤具有旺盛的增殖活性 ,是一个预后不良的信号。
Objective To investigate the expression of human telomerase reverse transcriptase (hTRT) in meningioma and to explore the relationship between hTRT and the malignancy of meningioma and prognosis. Methods The expression of hTRT and proliferating cell nuclear antigen (PCNA) in 43 cases of meningiomas was detected by immunohistochemistry. The positive rates of hTRT between different pathological grades of meningiomas and between primary and recurrent groups were compared. And compared the labeling index of PCNA between hTRT positive group and negative group. Results Among 43 meningioma cases, hTRT was positive in 19 cases, with a positive rate of 44%. The positive rates of hTRT in benign group, atypical group and malignant group were 115%, 90% and 100% respectively (P <0.01), and the positive rates of hTRT in recurrent group and primary group were 75% and 32% % (P <0.01). The PCNA labeling index of hTRT positive group and negative group were (38 4 ± 13 3)% and (11 8 ± 3 3)% respectively (P 0 01). Conclusion hTRT not only exists in atypical and malignant meningioma, but also exists in some morphologically benign meningiomas. The expression of hTRT is closely related to the malignancy of meningioma, which indicates that the tumor has strong proliferative activity, Is a poor prognosis signal.
