上颌窦粘液囊肿尚属少见,我科于1967～1994年收治鼻窦囊肿39例,其中粘液囊肿7例,均发生在上颌窦内.现将其临床资料报告如下.1 临床资料7例中,男5例,女2例;年龄18～50岁,30岁以下者5例.右侧4例,左侧3例.病史3月～7年,鼻塞、下鼻甲肥大、流脓性鼻涕5例;颇部麻木、头痛4例;颊部肿胀、局部隆起、触之有乒乓球感2例.7例患者均无鼻窦外伤及手术史.X线片示:囊肿界线清楚者3例,囊肿充满囊腔2例,有骨质破坏3例,1上颌窦腔内壁毛糙,密度增高不支持囊肿诊断.7例手术标本送检,大部分为形状不整囊壁组织及部分破碎组织.囊裂壁组织呈灰白色,囊内残留有粘浓样物质少许.镜下:囊壁结缔组织有增生,其间质毛细血管扩张充血及轻度水肿,并伴有散在炎细胞浸润,囊壁内层覆以假复层柱状纤毛上皮,部分呈立方状.
Maxillary sinus mucosal cyst is still rare, my department in 1967 to 1994, 39 cases of sinus cysts were treated, including 7 cases of mucinous cysts were occurred in the maxillary sinus. The clinical data are reported as follows.1 Clinical data 7 cases, male 5 cases and 2 females; aged 18 to 50 years old, 5 cases under the age of 30. 4 cases on the right side, 3 cases on the left side.History from March to 7 years, nasal obstruction, inferior turbinate hypertrophy, purulent nasal discharge in 5 cases; Partial numbness, headache in 4 cases; buccal swelling, local uplift, touch with ping-pong feeling in 2 cases .7 patients were no history of sinus surgery and surgery .X ray showed: cyst clear line in 3 cases, full of cysts 2 cases of cavity, 3 cases of bone destruction, 1 the maxillary sinus wall rough, high density does not support the diagnosis of cyst .7 cases of surgical specimens were sent for inspection, most of the shape of incomplete cyst wall tissue and some broken tissue. Gray, the capsule remains a thick sticky substance a little. Microscope: cyst wall connective tissue hyperplasia, interstitial telangiectasia congestion and mild edema, and accompanied by scattered inflammatory cell infiltration, the inner wall of the capsule covered with false Layer columnar ciliated epithelium, some were cubic.
