目的探讨芳香烃受体(AhR)在SLE患者外周血淋巴细胞中的变化及其与SLE疾病的关系,评估AhR在SLE疾病诊断中的价值。方法收集SLE患者20例和健康志愿者22例,采用流式细胞术及实时荧光定量PCR(q RT-PCR)方法对AhR的表达情况进行检测,并与SLEDAI-2K评分、ESR、补体C3和C4及CRP做相关性分析及二元Logistic回归分析。结果 SLE患者外周血AhR阳性淋巴细胞占淋巴细胞比值(10.76±4.52)%显著高于健康对照组(0.45±0.20)%;SLE患者外周血淋巴细胞中AhR mRNA表达水平(0.21±0.14)显著高于健康对照组(0.05±0.01),差异均有统计学意义(P<0.01)。外周血AhR阳性淋巴细胞占淋巴细胞比值与SLE患者血清补体C3(r=-0.53,P<0.05)、C4(r=-0.63,P<0.05)呈负相关,与ESR呈正相关(r=0.55,P<0.05),而与CRP(r=-0.36,P>0.05)和SLEDAI-2K评分(r=-0.06,P>0.05)无相关性。二元Logistic回归分析结果显示,AhR阳性淋巴细胞占淋巴细胞比值[EXP(B)=3.337,P=0.001]、ESR[EXP(B)=1.354,P=0.018]是SLE疾病的独立危险因素,且AhR阳性淋巴细胞占淋巴细胞比值相较于ESR统计学差异更显著。结论外周血淋巴细胞中的AhR可用于评估SLE疾病的活动性并可用于SLE疾病的诊断,淋巴细胞中高表达的AhR可能是促进SLE发病的重要因素。
Objective To investigate the changes of aromatic receptor (AhR) in peripheral blood lymphocytes of patients with SLE and its relationship with SLE and to evaluate the value of AhR in the diagnosis of SLE. Methods Twenty cases of SLE patients and 22 healthy volunteers were collected. The expression of AhR was detected by flow cytometry and real-time quantitative PCR (q RT-PCR), and was compared with those of SLEDAI-2K, ESR, Correlation analysis of C4 and CRP and binary logistic regression analysis. Results The ratio of AhR positive lymphocytes to lymphocytes in peripheral blood of SLE patients (10.76 ± 4.52)% was significantly higher than that of healthy controls (0.45 ± 0.20)%. The expression of AhR mRNA in peripheral blood lymphocytes of SLE patients (0.21 ± 0.14) was significantly higher In the healthy control group (0.05 ± 0.01), the difference was statistically significant (P <0.01). The ratio of AhR positive lymphocytes to lymphocytes in peripheral blood was negatively correlated with serum complement C3 (r = -0.53, P <0.05) and C4 (r = -0.63, P <0.05) , P <0.05), but no correlation with CRP (r = -0.36, P> 0.05) and SLEDAI-2K score (r = -0.06, P> 0.05). Binary logistic regression analysis showed that the ratio of AhR positive lymphocytes to lymphocytes [EXP (B) = 3.337, P = 0.001] and ESR [EXP (B) = 1.354, P = 0.018] were independent risk factors for SLE. And AhR-positive lymphocytes ratio of lymphocytes compared to the ESR statistical differences more significant. Conclusions AhR in peripheral blood lymphocytes can be used to assess the activity of SLE disease and can be used in the diagnosis of SLE disease. High expression of AhR in lymphocytes may play an important role in the pathogenesis of SLE.
