目的评估维持性血液透析患者(MHD)成纤维细胞生长因子23(FGF-23)与营养不良之间的相关性。方法测定60例MHD患者血清FGF-23、1,25-二羟维生素D3[1,25-(OH)2VitD3]和人体测量学指标:体重指数(BMI),肱三头肌皮褶厚度(TSF),上臂围(MAC),上臂肌围(MAMC);同时也检测C-反应蛋白(CRP)、血浆白蛋白(ALB)、转铁蛋白(TF)、总胆固醇(TC)、肌酐(SCr)、尿素氮(BUN)、血钙(Ca)、血磷(P)、全段甲状旁腺素(iPTH)、瘦素(leptin)及血红蛋白(Hb)。FGF-23与各营养指标之间的关系采用直线相关和回归分析。结果患者FGF-23与改量定量主观整体评分法(MQSGA)、P、CRP、iPTH呈正相关(P<0.05),FGF-23与BMI、TSF、MAMC、ALB、1,25-(OH)2VitD3呈负相关(P<0.05)。结论 FGF-23是影响血液透析患者营养状况的重要因素之一。
Objective To assess the association between MHD fibroblast growth factor 23 (FGF-23) and malnutrition in maintenance hemodialysis patients. Methods The serum levels of FGF-23, 1,25-dihydroxyvitamin D3 [1,25- (OH) 2VitD3] in 60 patients with MHD and the anthropometric parameters including body mass index (BMI), triceps skinfold thickness MAC, MAMC, CRP, ALB, TF, TC and SCr were measured at the same time. BUN, Ca, P, iPTH, leptin and hemoglobin (Hb) were measured. The relationship between FGF-23 and nutrition indicators was analyzed by linear correlation and regression analysis. Results FGF-23 was positively correlated with MQSGA, P, CRP and iPTH (P <0.05), FGF-23 was positively correlated with BMI, TSF, MAMC, ALB and 1,25- (OH) 2VitD3 Was negatively correlated (P <0.05). Conclusion FGF-23 is one of the important factors affecting the nutritional status of hemodialysis patients.
