目的探讨热休克凋亡的人胃癌细胞致敏的树突状细胞(DC)疫苗对胃癌术后免疫功能的影响。方法从胃癌患者外周血单个核细胞中诱导DC,并用重组人粒细胞-巨噬细胞集落刺激因子(rhGM-CSF)和白细胞介素4(rhIL-4)刺激活化,经热休克凋亡自体胃癌细胞致敏制备DC疫苗。将36例胃癌术后患者随机分为化疗加用DC瘤苗治疗(A)组18例,单纯化疗对照(B)组18例。对两组病例治疗前后T细胞亚群及自然杀伤(NK)细胞及血清IL-2、IL-12、γ干扰素(IFN-γ)水平进行比较。结果A组治疗后外周血CD3+、CD3+CD4+、CD4+/CD8+及NK细胞比率较治疗前明显升高(P<0.05),且明显高于B组(P<0.05);A组治疗后血清IL-2、IL-12、IFN-γ水平较治疗前明显升高(P<0.05)。结论胃癌术后行热休克凋亡自体胃癌细胞致敏的DC疫苗治疗可改善患者的免疫功能。
Objective To investigate the effect of dendritic cell (DC) vaccine sensitized by heat shock on human gastric cancer cells to postoperative immune function of gastric cancer. Methods DCs were induced from peripheral blood mononuclear cells of gastric cancer patients and activated by recombinant human granulocyte-macrophage colony-stimulating factor (rhGM-CSF) and interleukin-4 (rhIL-4) Cell Sensitization to Prepare DC Vaccine. Thirty-six patients with gastric cancer were randomly divided into chemotherapy plus DC vaccine (A group, 18 patients) and chemotherapy alone group (B), 18 patients. The levels of T-lymphocyte subsets, natural killer (NK) cells and serum levels of IL-2, IL-12 and IFN-γ in two groups before and after treatment were compared. Results The ratio of CD3 +, CD3 +, CD4 + / CD8 + and NK cells in peripheral blood of group A was significantly higher than that before treatment (P <0.05), and significantly higher than that of group B (P <0.05) -2, IL-12 and IFN-γ were significantly higher than those before treatment (P <0.05). Conclusions Heat shock and apoptosis of gastric cancer patients after autologous gastric cancer cells can improve the immune function of DC patients.
