目的:研究乙肝患者HBV基因型的分布及临床意义。方法:采集济南市传染病医院2007年1月-2009年12月乙肝病毒感染者579例,采用基因芯片进行HBV基因分型,生化指标在酶动力学法雅培C i8200全自动仪上测得。结果:在肝病患者中,B型99例(17.1%),C型468例(80.8%),B/C混合型12例(2.1%)。C型在肝病患者基因型中所占构成比最高,差异有显著性(P<0.01)。B型、C型及B/C型之间HBV DNA定量及ALT、AST水平差异均无显著性(P>0.05);不同基因型的CHE水平差异统计有显著性(P<0.01),B/C型最低,C型次之,B型最高。结论:乙肝患者其HBV基因型存在有B型、C型和B/C混合型;其中C型为优势基因型;不同基因型与肝脏的合成能力及预后有关,B型预后最好,B/C型最差。
Objective: To study the distribution and clinical significance of HBV genotypes in hepatitis B patients. Methods: 579 cases of hepatitis B virus infection were collected from Jinan Infectious Disease Hospital from January 2007 to December 2009, and HBV genotypes were detected by gene chip. The biochemical indexes were measured on the Abbott C i8200 automatic analyzer with enzyme kinetics. RESULTS: Among the patients with liver disease, 99 (17.1%) were type B, 468 (80.8%) were C and 12 (2.1%) were B / C mixed. C genotype in patients with liver disease accounted for the highest proportion, the difference was significant (P <0.01). There were no significant differences in the levels of HBV DNA and ALT, AST between B, C and B / C genotypes (P> 0.05). The differences of CHE levels among different genotypes were statistically significant (P <0.01) C type lowest, C type second, B type highest. Conclusion: There are B, C and B / C mixed HBV genotypes in hepatitis B patients, of which C genotype is the predominant genotype. The different genotypes are related to liver synthesis ability and prognosis, B type has the best prognosis, B / C is the worst.
