目的 探讨五种小儿常见疾病的甲皱微循环变化。方法 应用甲皱显微镜对 98例五种小儿常见疾病患儿的甲皱微循环变化进行观察 ,并与 70例正常儿童比较。结果 :①五种疾病甲皱微循环的各项积分值和总积分值均明显增高。血液病、肾综、急性肾炎、肺炎组与健康儿组比较差异显著 (P <0 .0 5~ 0 .0 0 1)。癫痫组与健康儿组比较 ,无显著性差异 (P >0 .0 5 )。②血液病组甲皱微循环的总积分值明显高于其余 4组 ,与小儿癫痫、肺炎、肾炎组比较差异显著 (P <0 .0 5~ 0 .0 1)。与肾综组比较无显著性差异 (P >0 .0 5 )。③五种疾病治疗后甲皱微循环各项积分值及总积分值较治疗前明显下降。血液病、急性肾炎、肾综 ,肺炎组治疗前后比较差异非常显著 (P <0 .0 1~ 0 .0 0 1)。癫痫组治疗前后比较无显著性差异 (P >0 .0 5 )。结论 五种小儿常见疾病存在不同程度的微循环障碍 ,且随病情进展 ,变化显著。血液病患儿微循环障碍明显。加用 6 5 4-2及活血化瘀中药治疗后 ,甲皱微循环障碍明显改善。提示开展甲皱微循环检查不仅有助于小儿常见疾病病情的判断与疾病预后的估计 ,而且可用以指导治疗
Objective To investigate the changes of nail fold microcirculation in five common diseases in children. Methods The changes of nail fold microcirculation in 98 children with common diseases of five kinds of children were observed by nail fold microscope and compared with 70 normal children. Results: ① The integrals and total scores of nail fold microcirculation in five diseases were significantly increased. Hematopathy, nephrotic syndrome, acute nephritis, pneumonia and healthy children were significantly different (P <0.05-0.0001). Epilepsy group and healthy children group, no significant difference (P> 0.05). ② The total score of nail fold microcirculation in blood disease group was significantly higher than the other 4 groups, which was significantly different from those in epilepsy, pneumonia and nephritis group (P <0.05-0.01). Compared with the renal comprehensive group no significant difference (P> 0.05). ③ After treatment of five diseases nail fold microcirculation points and the total value of points than before treatment decreased significantly. Blood disease, acute nephritis, renal comprehensive, pneumonia group before and after treatment was very significant difference (P <0.01 ~ 0.01). There was no significant difference in epilepsy group before and after treatment (P> 0.05). Conclusion There are some microcirculation disorders in five kinds of pediatric common diseases, and the changes are obvious with the progression of the disease. Hematologic disorders in children with microcirculation obvious. Plus 6 5 4-2 and blood circulation Chinese medicine treatment, nail fold microcirculation significantly improved. Tips to carry out nail fold microcirculation examination not only contribute to the diagnosis of common diseases in children with the prognosis of the disease, but also can be used to guide the treatment
