目的 :观察新生儿动脉导管未闭的自然闭合时间及不同情况下应采取的相应措施。方法 :应用ACUSON12 8XP/10彩超仪观察动脉导管的形态、粗细、长度、分流方向、时相及速度、分流压差 ,选择导管内径 2~ 4 3mm新生儿共 2 9例 ,3,6 ,12个月进行超声心动图追踪观察。结果 :自然闭合者 3个月内 2 5例 ,占86 2 % ;6个月 2例 ,占 6 9% ;12个月 1例 ,占 3 4% ;1例早产、低体重儿 ,动脉导管未闭合并房间隔小缺损 ,同时有吸入性肺炎并心衰 ,最后因心衰不能纠正而死亡。结论 :新生儿细小动脉导管未闭 ,特别在早产儿 ,随着时间推移多能自然闭合 ,当导管内径较宽和 (或 )合并心衰时 ,应及时采取相应措施促其闭合。
OBJECTIVE: To observe the natural closure time of patent ductus arteriosus and the corresponding measures in different situations. Methods: The shape, thickness, length, shunt direction, time phase and velocity of arterial catheter were observed with ACUSON12 8XP / 10 colorimeter. The pressure difference was divided into two groups. Echocardiography was followed up for months. Results: There were 25 cases of natural closure in 3 months, 86 2%; 6 cases in 6 cases, accounting for 69%; 12 cases in 1 case, accounting for 34%; 1 case of premature, low birth weight, Closure of atrial septal defect, with aspiration pneumonia and heart failure, and finally due to heart failure can not be corrected and died. CONCLUSIONS: Neonatal patent ductus arteriosus, especially in premature infants, can naturally close over time. When the catheter has a wide inner diameter and / or complicated heart failure, appropriate measures should be taken to close it.
