目的:回顾性分析52例儿童发育性髋关节脱位(DDH)治疗前后的影像学特征,为DDH治疗提供诊疗依据。方法:对52例DDH儿童共70髋运用数字平片、髋关节造影、三维CT及MRI扫描多种影像学方法进行检查,采用Zionts分级标准分为4度,结合术前及术后髋臼指数(AI)和股骨颈前倾角(FNA)变化,进行手术方式的选择及术前术后的评价。结果:48髋手术治疗,术前AI(35.87±2.09)度,治疗后AI(26.32±2.89)度;FNA术前(51.38±10.67)度,治疗后FNA(19.02±2.11)度,进行统计学分析比较,术前AI、FNA和术后的差异均具有统计学意义(P<0.01)。优良64髋关节,占91%。结论:运用现代多种影像学检查,为DDH治愈率的提高提供了精确的诊治依据。
Objective: To retrospectively analyze the imaging features of 52 children with developmental dislocation of the hip (DDH) before and after treatment, so as to provide the basis for the diagnosis and treatment of DDH. Methods: A total of 70 hips of 52 children with DDH were examined by digital imaging, hip arthroplasty, three-dimensional CT and MRI scans. The Zionts classification was divided into 4 degrees. Combined with preoperative and postoperative acetabular index (AI) and femoral neck anteversion (FNA) changes, the choice of surgical approach and preoperative and postoperative evaluation. Results: There were significant differences between the two groups (P> 0.05). RESULTS: The preoperative AI (35.87 ± 2.09), postoperative AI (26.32 ± 2.89), preoperative FNA (51.38 ± 10.67) and FNA after treatment (19.02 ± 2.11) Analysis and comparison, preoperative AI, FNA and postoperative differences were statistically significant (P <0.01). Excellent 64 hips, 91%. Conclusion: The use of a variety of modern imaging tests for the improvement of DDH cure rate provides an accurate diagnosis and treatment basis.
