目的了解新疆不同民族男性人群血清糖缺失性转铁蛋白(CDT)水平,评估CDT鉴别长期慢性酗酒男性人群的作用。方法采用问卷调查法确定131名正常男性(正常对照组,其中汉族53名、维吾尔族46名、哈萨克族32名)和96例长期慢性酗酒男性(长期酗酒组,其中汉族45例、维吾尔族31例、哈萨克族20例)为研究对象,采用毛细管电泳法检测血清CDT水平,然后通过P-P正态性检验确定CDT的参考区间。采用受试者工作特征(ROC)曲线确定CDT识别长期慢性酗酒者的临界值。结果长期酗酒组CDT水平为3.530%±0.833%,明显高于正常对照组(0.716%±0.341%,P<0.001)。2个组的组内不同民族之间CDT水平差异均无统计学意义(P>0.05)。初步确定新疆地区正常男性人群的血清CDT参考区间为<1.28%。ROC曲线分析显示CDT识别长期慢性酗酒者的临界值为1.25%,敏感性为81.3%,特异性为92.3%,曲线下面积为0.948。结论新疆地区少数民族血清CDT可以应用同一个参考区间。采用CDT识别长期慢性酗酒者具有较好的敏感性和特异性,可完全满足临床需要。
Objective To understand the level of serum glucose-deficient transferrin (CDT) in different ethnic groups in Xinjiang and evaluate the role of CDT in differentiating long-term chronic alcohol abuse male population. Methods A total of 131 normal men (control group, 53 Han, Uighur 46, Kazak 32) and 96 chronic alcohol abuse men (chronic alcohol abuse group, including 45 Han, Uighur 31 Case, Kazak 20 cases) as the research object, the serum CDT level was detected by capillary electrophoresis, and then the CDT reference interval was determined by PP normality test. The receiver operating characteristic (ROC) curve was used to determine the cut-off value of CDT in identifying chronic alcohol abusers. Results The level of CDT in chronic alcoholism group was 3.530% ± 0.833%, which was significantly higher than that in normal control group (0.716% ± 0.341%, P <0.001). There was no significant difference in CDT levels among different ethnic groups in the two groups (P> 0.05). Preliminary identification of normal male population in Xinjiang Serum CDT reference interval <1.28%. ROC curve analysis showed that the critical value of CDT in identifying chronic alcohol abusers was 1.25%, with a sensitivity of 81.3%, a specificity of 92.3% and an area under the curve of 0.948. Conclusion Serum CDT of ethnic minority in Xinjiang can use the same reference interval. The use of CDT to identify long-term chronic alcoholism has good sensitivity and specificity to fully meet the clinical needs.
