目的 探讨糖尿病患者尿中结缔组织生长因子 (CCN2 )浓度的变化及其意义。方法 将 3 0例糖尿病患者根据尿白蛋白排泄率 (UAER)分为正常白蛋白尿组 (Ⅰ组 )、微量白蛋白尿组(Ⅱ组 )和大量白蛋白尿组 (Ⅲ组 ) ,分别检测各组患者尿CCN2及临床生化指标 ,并与 10例对照组(NC组 )进行比较。结果 糖尿病各组患者尿人结缔组织生长因子 (CCN2 )含量显著高于对照组 (P<0 .0 5 ) ,且随着UAER增加其水平亦呈逐年递增趋势 ,组间比较差异有显著性 (P <0 .0 1)。相关分析表明 ,尿CCN2与UAER及呈显著正相关 (P <0 .0 1)与内生肌酐清除率 (Ccr)成负相关。结论 CCN2在糖尿病肾病 (DN)的发生发展中起着一定的作用。测定糖尿病患者尿CCN2的含量 ,可作为诊断DN早期肾损害的敏感指标之一 ,还将有助于监测、判断DN的病程进展
Objective To investigate the changes and clinical significance of urinary connective tissue growth factor (CCN2) in diabetic patients. Methods 30 cases of diabetic patients were divided into normal albuminuria group (group Ⅰ), microalbuminuria group (group Ⅱ) and large albuminuria group (group Ⅲ) according to urinary albumin excretion rate (UAER) The urinary CCN2 and clinical biochemical indexes of each group were compared with 10 control group (NC group). Results The urinary connective tissue growth factor (CCN2) in diabetic patients was significantly higher than that in the control group (P <0.05), and the level of CCN2 increased gradually with the increase of UAER. The difference was significant P <0 .0 1). Correlation analysis showed that there was a significant positive correlation between urine CCN2 and UAER (P <0.01) and endogenous creatinine clearance (Ccr). Conclusion CCN2 plays a role in the occurrence and development of diabetic nephropathy (DN). Determination of urinary CCN2 urinary contents of urine can be used as a sensitive indicator of early diagnosis of DN renal damage will also help monitor and determine the course of DN progression
