目的 :探讨P53表达在早期胃癌进展中的作用。方法 :用免疫组化法检测了 114例早期胃癌中P53及细胞周期蛋白 (Cyclin)E的表达。 结果 :正常胃腺上皮无阳性表达 ;胃癌中P53阳性表达位于癌细胞核内 ;P53总阳性率 3 1% ( 3 5 / 114 )P53表达率为男性 3 7% ( 3 1/ 83 ) ,组织学分化型 4 1% ( 3 3 / 80 ) ,均显著增高 ;此率在侵入粘膜下者是 4 0 % ( 2 1/ 5 3 ) ,侵润淋巴管者 4 2 % ( 14 / 3 3 ) ,侵入静脉者 4 7% ( 9/ 19) ,都倾向高于对照组。P53阳性癌的CyclinE表达率显著升高。P53阳性者 5年生存率 74 % ( 2 6/ 3 5 ) ,显著低下。结论 :P53表达很可能参与和促进了早期胃癌的进展 ,P53阳性的早期胃癌生物学恶性度高 ,须给予积极的治疗
Objective: To explore the role of P53 expression in the progression of early gastric cancer. Methods: The expression of P53 and cyclin E in 114 cases of early gastric cancer was detected by immunohistochemistry. Results: There was no positive expression of normal gastric gland epithelium; the positive expression of P53 in gastric cancer was located in the nucleus of cancer cells; the overall positive rate of P53 was 3 7% (3 5 / 114) P53 expression rate was 37% (31/83) in males, histological differentiation Type 4 1% (33/80) were significantly higher; this rate was 40% (2 1/5 3) in the submucosal invasion and 4 2% (14/ 3 3) invaded lymphatic vessels. Intravenous 47% (9/19) tend to be higher than the control group. The expression of CyclinE in P53-positive cancers was significantly increased. The 5-year survival rate of P53-positive patients was 74% (26/35), which was significantly lower. Conclusion: The expression of P53 may participate in and promote the progression of early gastric cancer. P53-positive early gastric cancer has a high degree of malignancy and must be given active treatment.
