目的了解贵阳市社区中老年人群慢性疼痛患病情况,为社区慢性疼痛医疗卫生服务提供科学依据。方法采取多阶段分层整群随机抽样,抽取7个社区的部分中老年人,采用自制问卷进行入户面对面询问调查。结果回收有效问卷2 091份,检出慢性疼痛患者863人,患病率为41.3%,95%CI(39.0%,43.4%);中年人患病率36.9%,95%CI(34.1%,39.5%),老年人患病率48.3%,95%CI(44.9%,51.9%)。男性患病率为35.1%,95%CI(31.7%,38.4%);女性患病率为45.5%,95%CI(42.9%,48.3%)。疼痛患者的平均疼痛程度得分4分(3,6),平均疼痛时间3.5年(1.5年,10年),疼痛最常见的3个部位依次是下肢、腰骶部和颈部。慢性疼痛患者的就诊率为58.6%;治疗后疼痛完全缓解率3.6%,部分缓解率84.8%,未缓解率11.5%。社区中老年慢性疼痛知晓率为14.3%。结论贵阳市社区中老年人群慢性疼痛患病率较高,疼痛患者就诊率较低,治疗后疼痛缓解率低,慢性疼痛的知晓率低。应关注社区慢性疼痛的预防、治疗及慢性疼痛知识的宣传。
Objective To understand the prevalence of chronic pain in the middle-aged and elderly population of Guiyang community and to provide a scientific basis for community chronic pain medical and health services. Methods A multistage stratified cluster random sampling method was used to select some middle-aged and elderly people in 7 communities. The self-made questionnaires were used to investigate face-to-face interviews in households. Results A total of 2 091 valid questionnaires were collected and 863 chronic pain patients were detected. The prevalence rate was 41.3%, 95% CI (39.0%, 43.4%). Middle-aged people were 36.9%, 95% CI (34.1% 39.5%), the prevalence of the elderly was 48.3%, 95% CI (44.9%, 51.9%). The prevalence of males was 35.1%, 95% CI (31.7%, 38.4%). The prevalence of females was 45.5%, 95% CI (42.9%, 48.3%). The average pain score of patients with pain was 4 points (3,6) and average pain time was 3.5 years (1.5 years, 10 years). The most common pain sites were lower limbs, lumbosacral region and neck. The treatment rate of patients with chronic pain was 58.6%. The pain relief rate was 3.6%, the partial response rate was 84.8% and the unresponsive rate was 11.5%. Community awareness of chronic pain in the elderly was 14.3%. Conclusion The prevalence of chronic pain is higher in middle-aged and elderly people in community in Guiyang. The visiting rate of pain patients is lower, the pain relief rate is lower and the awareness of chronic pain is lower. Should pay attention to the prevention and treatment of chronic pain in the community, and advocacy of chronic pain knowledge.
