目的应用多普勒超声心动图对左室心功能多项指标的检测,同时测定血浆脑型钠利尿肽(BNP)的浓度,对提高舒张性心力衰竭(DHF)诊断及其代表心功能状态和心脏结构多项指标的相关性。方法选择门诊及住院患者共142例,检测左室心功能多项指标及血浆BNP浓度,所有资料进行统计并作相关性分析。结果1.心功能正常组超声指标LVEDD、LVM、LVMI与BNP呈正相关(P<0.01)与EF、EV、EV/AV呈负相关(P<0.001);2.单纯性DHF组EDT、LVEDD、IVSD、LVPWD、LVM、LVMI、AV与BNP呈显著正相关(P<0.0001)与EF、IVRT、EV、EV/AV呈显著负相关(P<0.0001);3.舒张合并收缩性左室衰竭组EDT、LVEDD、LAD与BNP呈显著正相关(P<0.0001)与EF、IVRT呈显著负相关(P<0.0001)。结论1.三组中超声多普勒多项指标与血浆BNP均有相关性;2.随着超声指标与BNP水平升高和部分指标降低心衰严重程度逐渐加重。
Objective To evaluate the detection of multiple indexes of left ventricular cardiac function by Doppler echocardiography and to determine the concentration of plasma brain natriuretic peptide (BNP) in diastolic heart failure (DHF) Correlation of multiple indicators of cardiac structure. Methods A total of 142 outpatients and inpatients were selected for the study. Multiple indexes of left ventricular cardiac function and plasma BNP concentration were measured. All data were statistically analyzed and correlated. LVEDD, LVM, LVMI and BNP were positively correlated (P <0.01) with EF, EV and EV / AV in normal heart function group (P <0.001) There was a significant negative correlation between IVSD, LVPWD, LVM, LVMI, AV and BNP (P <0.0001) and EF, IVRT, EV and EV / AV There was a significant negative correlation between EDT, LVEDD, LAD and BNP (P <0.0001) and EF and IVRT (P <0.0001). 1.The three groups of ultrasound Doppler multiple indicators have correlation with plasma BNP; 2.With the ultrasound index and BNP levels increased and some indicators to reduce the severity of heart failure gradually increased.
