目的对胃黏膜pH对小儿重症肺炎合并胃黏膜损伤的临床诊断价值进行分析。方法选取本院在2012年4月~2014年1月收治重症肺炎并发胃黏膜损伤患儿52例为观察组,选取同期收治单纯重症肺炎患儿50例为对照组,对比两组患儿不同时间胃黏膜pH。结果观察组通气前胃黏膜pH及PaO2、PgCO2-PaCO2、PaO2/FiO2均显著低于对照组;在机械通气时间不断延长下,两组患者各指标均有所改善,且观察组各指标仍显著低于对照组;对观察组各指标进一步分析,PaO2、PaO2/FiO2与胃黏膜pH成正比,PgCO2-PaCO2与胃黏膜pH成反比。结论重症肺炎并发胃黏膜损伤的患儿胃黏膜pH相对较低,同时可用于对患者胃黏膜缺血缺氧改善情况评估之中,值得在临床中推广。
Objective To analyze the value of gastric mucosal pH in the diagnosis of severe pneumonia complicated with gastric mucosal injury in children. Methods 52 cases of severe pneumonia complicated with gastric mucosal injury in our hospital from April 2012 to January 2014 were selected as the observation group. Fifty cases of children with severe pneumonia were selected as the control group in the same period. Gastric mucosal pH. Results The gastric mucosal pH, PaO2, PgCO2-PaCO2 and PaO2 / FiO2 in the observation group before ventilation were significantly lower than those in the control group. Under the condition of prolonged mechanical ventilation, the indexes of both groups improved and the indexes in the observation group were still significant PaO2, PaO2 / FiO2 and gastric mucosal pH is proportional to, PgCO2-PaCO2 and gastric mucosal pH is inversely proportional to. Conclusions Gastric mucosal pH in children with severe pneumonia complicated with gastric mucosal injury is relatively low and can be used in the evaluation of gastric mucosal ischemia and hypoxia. It is worth to be popularized clinically.
