目的比较三孔法与五孔法腹腔镜乙状结肠癌根治术的临床可行性与安全性。方法 60例乙状结肠癌患者,按手术方法不同分为三孔组(给予三孔法腹腔镜乙状结肠癌根治术治疗,28例)和五孔组(给予五孔法腹腔镜乙状结肠癌根治术治疗,32例)。对比两组患者的手术时间、术中出血、肿瘤远切缘长度、肿瘤近切缘长度、清扫淋巴结个数、术后排气时间、术后住院时间、术后严重并发症(吻合口瘘、吻合口出血)发生情况。结果两组患者的手术时间、术中出血、肿瘤远切缘长度、肿瘤近切缘长度、清扫淋巴结个数比较,差异均无统计学意义(P>0.05)。三孔组患者的术后排气时间(2.4±1.2)d、术后住院时间(8.2±1.4)d、术后严重并发症(吻合口瘘0例、吻合口出血1例)与五孔组[(2.5±1.6)d、(8.6±1.7)d、(0例、1例)]比较,差异均无统计学意义(P>0.05)。结论三孔法腹腔镜乙状结肠癌根治术具备和传统五孔法同等的临床有效性及安全性,并且在减少腹部创伤及美容效果方面更具优势,更加符合微创手术理念,具有临床应用价值。
Objective To compare the clinical feasibility and safety of three-hole and five-hole laparoscopic radical resection of sigmoid colon cancer. Methods Sixty patients with sigmoid colon cancer were divided into three-hole group (three-hole laparoscopic radical resection of sigmoid colon cancer, 28 cases) and five-hole group (five-hole laparoscopic radical resection of sigmoid colon cancer, 32 example). The operative time, intraoperative bleeding, length of distal margin of tumor, length of tumor near margin, number of lymph nodes dissected, postoperative exhaust time, postoperative hospital stay, postoperative serious complications (anastomotic fistula, Anastomotic bleeding) occurred. Results There was no significant difference in operative time, intraoperative bleeding, length of distal margin of tumor, length of tumor near margin, number of lymph nodes dissected between the two groups (P> 0.05). In the three-hole group, the postoperative exhaust time (2.4 ± 1.2) days, postoperative hospital stay (8.2 ± 1.4) days, postoperative serious complications (0 cases of anastomotic fistula and 1 case of anastomotic bleeding) There were no significant differences between the two groups (P> 0.05). There was no significant difference between the two groups (P> 0.05). Conclusions The three-hole laparoscopic radical resection of sigmoid colon carcinoma has the same clinical efficacy and safety as the traditional five-hole method, and has more advantages in reducing abdomen trauma and cosmetic effects, which is more in line with the concept of minimally invasive surgery and has clinical value.
