目的探讨阻塞性睡眠呼吸暂停综合征(obstructive sleep apnea syndrome,OSAS)患者的颌下腺。方法回顾46例摘除颌下腺治疗OSAS的颌下腺资料,包括患者颌下腺病史综合与体检,以及颌下腺影像学研究,术中观察到的颌下腺形态和走向、体积测量,病理学检查,并与正常颌下腺进行对照,研究探讨OSAS患者的颌下腺。结果症状:46例患者均有咽喉部紧缩梗阻感,常有呼吸不畅,睡眠时打鼾,憋气;口底颌下区两侧触及颌下腺肿大,质地均匀较软;影像学:CT和MRI均显示颌下腺在下颌骨下缘以下;颌下腺最小者长度为3.5cm,宽3cm。体积:24~40cm3;病理检查:排除肿瘤,腺泡细胞测量直径均值为:58.56μm。结论OSAS患者均有颌下腺向下移位并肿大,即颌下腺下垂,是一种病变。
Objective To investigate the submandibular gland in patients with obstructive sleep apnea syndrome (OSAS). Methods The data of submandibular gland of 46 patients with submandibular gland extirpation were retrospectively reviewed, including the comprehensive and physical examination of the submandibular gland, the submandibular gland imaging, the submandibular gland morphology and orientation, the volume measurement and the pathological examination. Conclusion All submandibular glands in patients with OSAS have downward displacement and enlargement, that is, submandibular gland drooping, which is a kind of lesion.
