目的:比较针刺星状神经节配合静脉滴注前列地尔与单纯静脉滴注前列地尔治疗早期糖尿病下肢动脉粥样硬化的疗效差异。方法:将60例早期糖尿病下肢动脉粥样硬化患者随机分为观察组和对照组,每组30例。两组均予调控血糖、血脂等基础治疗,对照组采用前列地尔10μg加入0.9%NaCl溶液100mL静脉滴注;观察组在对照组治疗基础上加用针刺星状神经节治疗。两组均每日治疗1次,连续治疗2周为一疗程,共治疗4个疗程。观察两组患者治疗前后证候总积分、糖化血红蛋白(HbA1c)、胫后及足背动脉血流量,并比较两组临床疗效。结果:两组治疗后证候总积分、HbA1c、胫后及足背动脉血流量均较治疗前改善,治疗前后差异均具有统计学意义(均P<0.05),且观察组治疗后各项指标的改善均优于对照组,组间差异均具有统计学意义(均P<0.05);治疗后观察组总有效率为93.3%(28/30),优于对照组的86.7%(26/30,P<0.05)。结论:针刺星状神经节配合静脉滴注前列地尔治疗早期糖尿病下肢动脉粥样硬化疗效优于单纯静脉滴注前列地尔,可改善患者临床症状,调控血糖水平,增加下肢血流量。
OBJECTIVE: To compare the efficacy of acupuncture with stellate ganglion and intravenous infusion of alprostadil and simple alprostadil in the treatment of atherosclerosis of lower extremities. Methods: Sixty patients with early-stage diabetic lower extremity atherosclerosis were randomly divided into observation group and control group, with 30 cases in each group. The two groups were to be controlled by blood sugar, blood lipids and other basic treatment. The control group was treated with 100 microliters of alprostadil plus 0.9% NaCl solution intravenously. The observation group was treated with acupuncture of stellate ganglion on the basis of the control group. Two groups were treated once a day for two weeks for a course of treatment, a total of four courses of treatment. The total score of syndromes, HbA1c, posterior tibial and dorsalis pedis artery blood flow in both groups were observed before and after treatment. The clinical curative effect was compared between the two groups. Results: After treatment, the total score of syndromes, blood flow of HbA1c, posterior tibial and dorsalis pedis were all improved compared with those before treatment, and the difference was statistically significant before and after treatment (both P <0.05) (P <0.05). After treatment, the total effective rate in the observation group was 93.3% (28/30), which was better than that in the control group (86.7%, 26/30 , P <0.05). Conclusion: Acupuncture stellate ganglion combined with intravenous alprostadil treatment of early diabetic lower extremity atherosclerosis better than simple intravenous infusion of alprostadil, can improve clinical symptoms, regulate blood sugar levels and increase lower extremity blood flow.
