本实验采用三维超声心动图重建技术(3DE)测量开胸的10条成年杂种犬的左室容量,并与二维超声心动图(2DE)和电磁流量计(EMF)的主动脉流量测值对照。用3.75MHz探头心尖部旋转法获取一系列间隔为30°的高分辨力图像。结扎冠状动脉前降支中部,半小时后重复测量。人为描记心内膜,将边缘坐标输入计算机进行三维重建,并计算左室容量。结果:冠脉结扎后,3DE及2DE的舒张末期容量(EDV)、收缩末期容量(ESV)测值均增大(P<0.05),3DE的每搏量(SV)明显下降(P<0.001)。这与EMF测值高度相关,2DE的SV值下降不明显(P>0.1),且与EMF无良好相关。冠状动脉结扎前,3DE与2DE测值相关良好,冠状动脉结扎后,二者相关系数减低,ESV相关性尤差(r=0.347)。结论:心肌梗塞后,尤其是收缩期心腔几何形状发生较大变化时,3DE对于心室容积测量的精确性优于2DE。
In this study, left ventricular volume was measured in three thoracotomy adult dogs by three-dimensional echocardiographic reconstruction (3DE), and compared with 2DE and EMF aortic flow measurements . A series of high resolution images spaced 30 ° apart were obtained using a 3.75 MHz probe apical rotation method. Ligation of the anterior descending coronary artery central, half an hour after repeated measurements. Artificial endocardial tracing, the edge of the coordinates input to the computer for three-dimensional reconstruction, and calculate the left ventricular volume. Results: After coronary ligation, the end-diastolic volume (EDV) and end-systolic volume (ESV) of 3DE and 2DE increased (P <0.05) and the stroke volume of 3DE decreased significantly (P < 0.001). This is highly correlated with EMF measurements, with no significant decrease in SV of 2DE (P> 0.1) and no good correlation with EMF. The correlation between 3DE and 2DE was good before coronary artery occlusion. The correlation coefficient between the two groups was decreased after coronary artery ligation, especially for ESV (r = 0.347). Conclusions: 3DE is superior to 2DE in the accuracy of ventricular volume measurement after myocardial infarction, especially in systolic cardiac geometry.
