皮质下动脉硬化性脑病(SAE)又名Binswanger病,系一种发生在老年人脑深部穿支小动脉硬化造成的白质、基底节、桥脑和小脑白质等处多灶性“腔隙状态”,而皮质和脊髓不受损害。临床上以慢性高血压动脉硬化、痴呆和脑室周围白质广泛低密度灶为其特征。由于老年人SAE缺乏特异性临床症征,仅根据病理作出诊断,七十年代以前国外仅有散在病例报道,因此认为十分罕见。近年来随着人口寿命延长,CT普及和MRI的应用使老年人SAE在生前能作出诊断,从而激起了人们对老年人SAE关注。
