目的分析中青年急性心肌梗死(acute myocardial infarction,AMI)猝死患者的血液学指标特征。方法年龄<60岁AMI患者127例,61例猝死者为猝死组,治疗后病情稳定者66例为对照组。比较2组基本资料及入院24h内实验室检查结果。结果猝死组年龄[(54.1±4.9)岁]、肌红蛋白[(6.0±1.0)mg/L]、氨基末端脑钠肽前体含量[(8.3±1.3)ng/L]、白细胞计数[(13.0±6.3)×109/L]、谷丙转氨酶[(4.5±1.1)u/L]、血肌酐[(4.7±0.7)μmol/L]、三酰甘油[(2.0±1.4)mmol/L]、低密度脂蛋白[(2.8±1.3)mmol/L]高于对照组[年龄(50.0±7.4)岁、肌红蛋白(5.5±1.2)mg/L、氨基末端脑钠肽前体(7.3±1.6)ng/L、白细胞计数(11.8±5.1)×109/L、谷丙转氨酶(4.1±0.9)u/L、血肌酐(4.4±0.3)μmol/L、三酰甘油(1.6±0.9)mmol/L、低密度脂蛋白(2.7±0.9)mmol/L](P<0.05),男性比例(73.77%)、血红蛋白[(127.2±28.7)g/L]、血清白蛋白[(34.5±6.2)g/L]低于对照组[男性比例78.79%、血红蛋白(137.5±19.4)g/L、血清白蛋白(39.3±4.6)g/L](P<0.05)。结论年龄、性别及血液生化指标的变化可为AMI患者预后提供预测依据。
Objective To analyze the hematological characteristics of middle-aged and young people with sudden myocardial infarction (AMI) sudden death. Methods A total of 127 AMI patients aged 60 years or older were randomly divided into sudden death group and sudden death group. Sixty-six patients with stable disease after treatment were selected as the control group. The basic data of two groups were compared with the results of laboratory tests within 24 hours after admission. Results The age of sudden death group was (54.1 ± 4.9) years old, myoglobin [(6.0 ± 1.0) mg / L), amino terminal pro brain natriuretic peptide content [(8.3 ± 1.3) ng / L) (4.5 ± 1.1) u / L], serum creatinine [(4.7 ± 0.7) μmol / L] and triglyceride [(2.0 ± 1.4) mmol / L] (2.8 ± 1.3) mmol / L] were higher than those in the control group [50.0 ± 7.4 years of age, 5.5 ± 1.2 mg / L of myoglobin and 7.3 ± (1.6 ± 0.9) u / L, serum creatinine (4.4 ± 0.3) μmol / L, triglyceride (1.6 ± 0.9) mmol / (P <0.05), the proportion of males (73.77%), hemoglobin [(127.2 ± 28.7) g / L] and serum albumin [(34.5 ± 6.2) g / L] was lower than that of the control group [male ratio 78.79%, hemoglobin (137.5 ± 19.4) g / L, serum albumin (39.3 ± 4.6) g / L] Conclusion The changes of age, sex and blood biochemical indexes may provide the basis for prognosis of patients with AMI.
