目的 探讨二维斑点追踪显像技术(STI)评价肥厚性心肌病(HCM)患者左心房收缩和舒张功能改变的价值。方法 20例肥厚性心肌病患者(HCM组)和22例健康志愿者(对照组),应用STI技术分别获取左房前壁、后壁、侧壁、下壁、房间隔的基底段、中间段、心底段的应变率曲线。结果与对照组比较,HCM组左房各壁各节段收缩期应变率(SRs)、舒张早期应变率(SRe)和舒张晚期应变率(SRa)均显著减低;对照组左房各壁心底段的SRs、SRe、SRa均高于基底段;对照组和HCM组的SRs与SRe、SRa呈负相关;HCM组SRe与LVMI(左室质量指数)呈显著负相关。结论肥厚性心肌病左房各壁应变率峰值减低,且运动规律消失,代偿关系尚存在。
Objective To investigate the value of two-dimensional speckle tracking imaging (STI) in evaluating left atrial systolic and diastolic function in patients with hypertrophic cardiomyopathy (HCM). Methods Twenty patients with hypertrophic cardiomyopathy (HCM group) and 22 healthy volunteers (control group) were enrolled in this study. The anterior wall, posterior wall, posterior wall, inferior wall, atrial septum, , Heart section of the strain rate curve. Results Compared with the control group, the systolic strain rate (SRs), early diastolic strain (SRe) and late diastolic strain rate (SRa) in each segment of left atrium in HCM group were significantly decreased. In the control group, SRs, SRe, and SRa were all higher than those in the basal ganglia; SRs were negatively correlated with SRe and SRa in control group and HCM group; and SRe was negatively correlated with LVMI (left ventricular mass index) in HCM group. Conclusions The peak strain rate of left atrium in hypertrophic cardiomyopathy is decreased, and the motion rule disappears. The compensatory relationship still exists.
