目的探讨慢性阻塞性肺疾病(COPD)患者肺部CT灌注成像的时间密度曲线(TDC)和灌注参数特征。方法共44例,分为COPD组25例及对照组19例。均行肺部多层螺旋CT(MSCT)动态增强扫描。图像经后处理,设定感兴趣区(右腹肺、右背肺和左腹肺、左背肺),得到各处肺组织TDC和灌注参数,包括灌注值(P),强化峰值增量(PEI),达峰值时间(TTP)和血容量(BV)。比较分析COPD组和对照组的TDC特征,以及各灌注参数的差异性。结果COPD组较对照组TDC波峰延后。各灌注参数腹背肺比率在2组无显著性差异(P>0.05)。COPD组TTP较对照组明显延长(P=0.010)。P、PEI和BV在2组无显著性差异(P>0.05)。结论CT灌注成像显示COPD患者肺部血流灌注异常主要表现为TDC波峰延后,TTP延长。
Objective To investigate the time-density curve (TDC) and perfusion parameters of pulmonary CT perfusion imaging in patients with chronic obstructive pulmonary disease (COPD). Methods A total of 44 cases, divided into COPD group 25 cases and control group 19 cases. All patients underwent multislice spiral CT (MSCT) dynamic contrast-enhanced scanning. After image processing, the region of interest (right ventral lung, right back lung, left ventral lung, left back lung) was set up to obtain TDC and perfusion parameters of lung tissue, including perfusion value (P), enhanced peak increment PEI), peak time (TTP) and blood volume (BV). The characteristics of TDC in COPD group and control group were compared and the differences of perfusion parameters were analyzed. Results The TDC peak of COPD group was delayed than that of control group. There was no significant difference in perfusion parameters between the two groups (P> 0.05). TTP in COPD group was significantly longer than that in control group (P = 0.010). P, PEI and BV no significant difference in the two groups (P> 0.05). Conclusion CT perfusion imaging showed that pulmonary perfusion abnormalities in patients with COPD were mainly delayed TDC and prolonged TTP.
