目的 评估磁共振灌注成像 (PWI)在脑转移瘤伽玛刀疗效中的应用价值。方法 2 2例脑转移瘤伽玛刀治疗前及治疗后 3、6个月以上行PWI。得到相对局部脑血容量 (rCBV)图和对比剂平均通过时间 (MTT)图 ,计算病灶rCBV和MTT比率。结果 2 2例脑转移瘤的rCBV值均升高 ,肿瘤rCBV比率为 6.16± 3 .12 ,健侧为 1.18± 0 .17;肿瘤MTT比率为 1.44± 0 .5 3。 9例治疗后rCBV比率下降 ,MTT比率无变化 ,其中 3例证实为放射性脑坏死。 13例治疗后rCBV比率不同程度升高 ,其中 6例证实为肿瘤伴部分坏死组织。结论 PWI可作为评估伽玛刀疗效的一项重要指标
Objective To evaluate the value of perfusion magnetic resonance imaging (PWI) in the treatment of brain metastases with gamma knife. Methods Twenty-two patients with brain metastases underwent gamma knife treatment and received PWI more than 3,6 months after treatment. Relative local cerebral blood volume (rCBV) maps and mean contrast time (MTT) maps were obtained to calculate the rCBV and MTT ratios. Results The rCBV values of 2 cases of brain metastases were all increased. The rCBV ratio of tumor was 6.16 ± 3.12, and the contralateral ratio was 1.18 ± 0.17. The tumor MTT ratio was 1.44 ± 0.53. After treatment, the rCBV rate decreased and the MTT rate did not change, of which 3 cases were confirmed as radioactive brain necrosis. Thirteen cases of rCBV increased after treatment to varying degrees, of which 6 cases confirmed with tumor necrosis tissue. Conclusion PWI can be used as an important index to evaluate the efficacy of gamma knife
