目的观察胰岛素样生长因子1(IGF-1)对慢性压迫性脊髓损伤后神经功能恢复的影响.方法50只Wistar雌性大鼠,随机分为正常组、慢性重度(60%)压迫组、减压组、生理盐水(NS)组及IGF-1组.正常组不做任何处理,其余各组置入平头塑料螺钉对大鼠脊髓进行后路渐进性压迫,于2个月后达到60%左右重度压迫程度,然后减压组、NS组和IGF-1组彻底减压,NS组和IGF-1组分别于减压后1、3、5、7、9d在蛛网膜下腔给予NS和IGF-1(25μl),并于减压后2、4、6、8、10d后肢运动功能评分和检测运动诱发电位(MEP).结果压迫组大鼠后肢运动功能障碍,MEP波幅(P1~N1峰峰值)降低、潜伏期延长;IGF-1组大鼠后肢运动功能及MEP波幅及潜伏期有明显的恢复(P<0.05),且于4~6d后显著优于单纯减压组和NS组(P<0.05).结论IGF-1可明显促进慢性压迫性脊髓损伤后大鼠后肢运动功能及MEP波形的恢复.
