目的探讨急性期血糖值对动脉瘤性蛛网膜下腔出血(SAH)患者预后的影响。方法选取动脉瘤性SAH并行栓塞术的患者61例,按入院时及入院后1、2周的空腹血糖(FPG)值分为三组:A组19例,入院时及2周后FPG≥7.2mmol/L;B组20例,入院时FPG≥7.2mmol/L,2周内FPG恢复正常;C组22例,入院时FPG<7.2mmol/L。观察三组患者血管痉挛的发生率、出院时GOS评分及平均住院时间。结果 A组血管痉挛发生率高,GOS评分高,住院时间长,与B、C组比较,差异有统计学意义(P<0.05)。结论动脉瘤性SAH患者急性期血糖值升高是导致预后不良的危险因素。
Objective To investigate the effect of acute phase blood glucose level on the prognosis of patients with aneurysmal subarachnoid hemorrhage (SAH). Methods Sixty-one patients undergoing aneurysmal SAH with concurrent embolization were divided into three groups according to their fasting blood glucose (FPG) at admission and 1, 2 weeks after admission: group A (19 cases), FPG≥7.2 mmol / L; group B, 20 cases, admission FPG≥7.2mmol / L, 2 weeks FPG returned to normal; 22 cases in group C, FPG <7.2mmol / L admission. The incidence of vasospasm, the GOS score at discharge, and the average hospital stay were observed in the three groups. Results In group A, the incidence of vasospasm was high, the GOS score was high, and the length of hospital stay was longer. Compared with those in groups B and C, the difference was statistically significant (P <0.05). Conclusion Aneurysmal SAH patients with elevated blood glucose levels in the acute phase lead to poor prognosis risk factors.
