例1:张某,女,52岁,因右胁痛胀不舒,纳差半月就诊。查体:急性病容,肝大剑下3cm,肋下2cm,质韧触痛。实验室检查:ALT:248U/L,γ-GT:185U/L,HBsAg(-),HBsAb(+),HBeAg(-),HBeAb(-),HBcAb(-)(放免法)。病史,半月前因上呼吸道感染,在某医疗单位静脉点滴环丙沙星400mg,每日1次,连用5天后逐渐出现上述症状,既往无肝炎病史及长期大量饮酒史,曾于3年前注射乙肝疫苗,故考虑为环丙沙星所致之肝功能损害。 用药处方:柴胡、虎杖各12g,青黛9g(包煎),夏枯草、败酱草、丹参、构杞各15g,白茅根、茵陈各30g,栀于9g,甘草6g,水煎服,每日1剂,12剂后复查肝功能正常,随访1年未复发。
