随着经尿道前列腺电切术的广泛开展,对经尿道电切综合症(TURS)的认识日益清楚。产生TURS的根本原因是灌洗液的吸收,而影响灌洗液吸收的主要因素是膀胱及前列腺窝内压力。生理状况下盆腔静脉的内压力约10mmHg,排空后的膀胱内压力与此压力相同,但常规方式灌洗下行TURS膀胱内压常超过静脉压5~12倍,因而造成灌洗液不断从被切开的静脉吸收入血液循环,大量灌洗液的吸收将导致TURS的发生。低压灌洗可以减少灌洗液的吸收从而防止TURS发生。本文着重介绍低压灌洗方式的发展与现状。
With the extensive implementation of transurethral resection of the prostate, awareness of transurethral resection syndrome (TURS) has become increasingly clear. The root cause of TURS is the absorption of lavage fluid, and the main factor affecting the absorption of lavage fluid is the pressure in the bladder and prostate fossa. Under physiological conditions, the internal pressure of the pelvic veins is about 10mmHg, and the pressure inside the bladder after emptying is the same as that of the pressure. However, the conventional method of lavage of the bladder TURS pressure often exceeds 5 ~ 12 times the pressure of the venous pressure, The incised vein absorbs into the bloodstream and the absorption of large volumes of perfusate will result in TURS. Low-pressure lavage can reduce the absorption of lavage fluid to prevent TURS. This article focuses on the development and status quo of low pressure lavage.
