自1983年发现幽门螺杆菌(HP)以来,经学者们10年的研究,已确认该菌是慢性活动性胃炎和消化性溃疡的主要病因,胃的免疫在疾病发生中的作用亦引起了人们的关注,国内这方面的研究很少,所涉及的免疫细胞亦不多.本研究不仅调查了正常和HP~+慢性活动性胃炎胃窦部11种免疫细胞的分布和数量的变化,而且对18例胃粘膜相关淋巴组织(MALT)及淋巴小结(LF)的发生率进行了统计,并对LF内的免疫细胞进行了研究.结果:1.LF的发生率.正常组(HP-6例)偶见淋巴细胞(LC)聚集成堆,这些细胞以T总细胞为主,无一例出现LF.HP~+胃炎组(12例)胃粘膜内可见多处CD_3~+TC聚集成团的现象,7例(58.3%)出现LF.LF的出现与HP的数量有关,HP感染强阳性者(卅)均出现LF,其中2例呈现2~4个LF密集排列.大部分LF位于固有膜的浅层.2.LF的形态及免疫细胞定性研究.所见LF形态各异人F内的LC以 CD_3~+T总细胞为主,CD_3~+细胞主要位于相当于生发中心的周围部分(M区),LF中央(C区)较少.用抗CD_4、CD_8、CD_(57)、CD_(58)和CD_(20)单抗进一步证实,LF内的TC主要是CD_4~+细胞,而CD_3~+细胞主要位于C区,数量极少,在LF内还可见数量较多的NKC、单核细胞和BC.进一步用抗细胞因子单抗检测.发现LF内还有IL-2、IL-8和IFN分泌细胞.结论:HP~+慢性胃炎胃粘膜出现大量MALT,?
