1 临床特点①老年,女性,缓慢起病,急性加重.②有慢性腹痛,大便次数多,粘液血便病史.③有消瘦,贫血症状.④转移性右下腹痛,伴压痛反跳痛,局限性腹肌紧张,发热、白血球增高.2 诊断①急性化脓性阑尾炎.②直肠上段癌.③不全性肠梗阻.④中度贫血.还需做的必要特殊检查有:钡灌肠、纤维结肠镜检、乙状结肠镜检.本病例患者属急腹症,疑似阑尾炎、肠穿孔、局限性腹膜炎及不全性肠梗阻存在,此三项检查不适宜做.
1 Clinical characteristics 1 Older age, female, slow onset, acute exacerbation. 2 Chronic abdominal pain, frequent bowel movements, history of mucoblastic bloody stools. 3 Symptoms of weight loss and anemia. 4 Metastatic right lower quadrant pain, tenderness, rebound tenderness, limitations Abdominal muscle tension, fever, increased white blood cells. 2 Diagnosis 1 acute suppurative appendicitis. 2 Upper rectal cancer. 3 Incomplete intestinal obstruction. 4 moderate anemia. Necessary special examinations are needed: barium enema, colonoscopy, Sigmoid colonoscopy. This patient is an acute abdomen, suspected appendicitis, intestinal perforation, peritonitis, and incomplete intestinal obstruction, these three tests are not suitable.
