目的观察双水平气道正压通气对慢性阻塞性肺疾病夜间低氧的治疗作用。方法取60例慢性阻塞性肺疾病患者,随机分成2组。对照组30例使用吸氧、抗感染等常规治疗;治疗组在常规治疗的基础上加用夜间双水平气道正压通气治疗4 h,疗程为7 d。分别观察2组在治疗前后低氧血症及心率失常发生率和肺动脉压力的变化。结果双水平气道正压通气组在治疗前后自身对照以及与对照组比较,夜间低氧血症及心律失常发生率和肺动脉压力的改变均有统计学意义(P<0.05~0.01)。结论双水平气道正压通气能改善慢性阻塞性肺疾病患者的夜间低氧血症,减少心律失常的发生率,降低肺动脉压力,可作为治疗慢性阻塞性肺疾病的重要方法之一。
Objective To observe the therapeutic effect of bi-level positive airway pressure on nocturnal hypoxia in patients with chronic obstructive pulmonary disease. Methods Sixty patients with chronic obstructive pulmonary disease were randomly divided into two groups. The control group received conventional therapy such as oxygen inhalation and anti-infective. The treatment group was treated with nighttime double-level positive airway pressure ventilation for 4 hours on the basis of routine treatment for 7 days. The changes of hypoxemia and arrhythmia and pulmonary artery pressure were observed before and after treatment in two groups. Results Compared with the control group before and after treatment, the incidences of nocturnal hypoxemia, arrhythmia and pulmonary arterial pressure were significantly different (P <0.05 ~ 0.01). Conclusions Bi-level positive airway pressure ventilation can improve nocturnal hypoxemia in patients with chronic obstructive pulmonary disease, reduce the incidence of arrhythmia and decrease pulmonary artery pressure, which may be one of the important methods for the treatment of chronic obstructive pulmonary disease.
