目的 建立胎儿窘迫的妊娠羊动物模型并观测胎羊窘迫的动态过程。方法 妊娠晚期绵羊 (湖羊 ) 16头 ,于妊娠 116~ 12 5d进宫腔行胎儿手术 ,胎羊股动脉插管 ,使导管顶端达胎羊腹主动脉水平 ,留置导管于胎羊体内 ,经三通管接压力感受器 ,连续动态监测胎羊血压 ,定期采血。手术成功后经胎羊腹主动脉注射明胶微球颗粒 ,阻塞胎盘建立胎儿窘迫的动物模型。每次注射微球前后胎羊采血作血气分析 ,留置血清检测血乳酸、高密度脂蛋白 (HDL)和血糖等。结果 16例中 10例手术成功 ,经胎羊腹主动脉注射微球后 ,pH、pco2 、和 po2 与微球注射的次数 (MS)均有明显的关系 ,相关系数分别为 - 0 .72 40 (P <0 .0 1)、0 .5 2 15 (P <0 .0 5 )和 - 0 .70 0 79(P <0 .0 1) ;乳酸、血糖和HDL与微球注射次数之间相关性分别是 0 .5 0 35 (P <0 .0 5 )、0 .2 5 71(P >0 .0 5 )和 0 .1412(P >0 .0 5 )。乳酸浓度与pH值之间有明显的相关性 ,pH值 7.0以下时 ,乳酸浓度明显升高。 结论 经胎羊腹主动脉注射微球的方法 ,可以定量地表达胎盘阻塞的程度 ,符合胎儿窘迫的临床表现 ;该模型可动态地观察胎儿窘迫的过程。
Objective To establish a fetal animal model of fetal distress and to observe the dynamic process of fetal distress. Methods Twenty-six sheep (Hu sheep) in the third trimester of pregnancy were enrolled in the uterine cavity during 116-125 days of gestation. The femoral artery of the fetus was intubated. The top of the catheter reached the level of abdominal aorta of the fetus. The catheter was placed in the body of the fetus. Three-way tube pressure receptors, continuous dynamic monitoring fetal sheep blood pressure, regular blood collection. After successful operation, the animals were injected with gelatin microspheres through abdominal aorta and the placenta was occluded to establish animal model of fetal distress. Before and after each injection of microspheres blood for blood analysis of sheep, blood serum lactic acid, high density lipoprotein (HDL) and blood glucose and other tests. Results Of the 16 cases, 10 cases were operated successfully. After injecting microspheres via abdominal aorta, pH, pco2, and po2 were significantly correlated with the number of microspheres injected (MS), and the correlation coefficients were -0.72 (P <0. 01), 0 .5 2 15 (P <0. 05) and -0.70 0 79 (P <0. 01). The difference between lactic acid, blood glucose, HDL and microsphere injection times The correlations were 0.5035 (P <0.05), 0.52 (P> 0.05) and 0.1412 (P> 0.05), respectively. There is a clear correlation between the concentration of lactic acid and pH value. When the pH value is below 7.0, the concentration of lactic acid is obviously increased. Conclusion The method of injecting microspheres via abdominal aorta can quantitatively express the extent of placenta obstruction and meet the clinical manifestations of fetal distress. The model can dynamically observe the process of fetal distress.
