目的检测Ⅰ型自身免疫性肝炎(autoi mmune hepatitis typeⅠ,AI H-Ⅰ)患者增殖诱导配体(aproliferation-inducing ligand,APRIL)和B淋巴细胞刺激因子(B-lymphocyte activating factor,BAFF)mRNA水平,研究APRIL和BAFFmRNA的表达与AI H-Ⅰ临床特征之间的关系。方法半定量PCR法检测37例Ⅰ型AI H确诊患者(Ⅰ型AI H组)、19例乙型肝炎患者(乙型肝炎组)、15例正常人(正常对照组)外周血单核细胞APRIL和BAFFmRNA的表达水平,同时检测Ⅰ型AI H患者外周血中IgG、C3、C4、抗核抗体(ANA)、抗平滑肌抗体滴度(ASMA)、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、球蛋白(GLO)、C反应蛋白(CRP)水平。结果APRIL、BAFFmRNA在Ⅰ型AI H组的表达水平(0.65±0.39,0.76±0.35)显著高于乙型肝炎组(0.33±0.26,0.37±0.24)和正常对照组(0.30±0.25,0.23±0.20)。APRIL和BAFFmRNA表达水平在Ⅰ型AI H、乙型肝炎组、正常对照组中均呈中等强度的正相关(r=0.418,P<0.05)。APRIL、BAFFmRNA表达水平与疾病活动度指标(ALT、AST、GLO、IgG、CRP)、肝组织学炎症与纤维化评分之间无明显相关(P>0.05)。结论Ⅰ型AI H患者的APRIL、BAFFmRNA的表达水平显著升高,其水平与患者的预后有一定的相关性。
Objective To detect the expression of aproliferation-inducing ligand (APRIL) and B-lymphocyte activating factor (BAFF) mRNA in patients with autoimmune hepatitis typeⅠ (AI H-Ⅰ) To study the relationship between the expression of APRIL and BAFF mRNA and the clinical features of AI H-Ⅰ. Methods The expression of APRIL in 37 patients with type Ⅰ AI H (type Ⅰ AI H), 19 patients with hepatitis B (hepatitis B) and 15 normal controls (normal control group) were detected by semi-quantitative PCR. (ANA), anti-smooth muscle antibody titer (ASMA), alanine aminotransferase (ALT), aspartate Aminotransferase (AST), globulin (GLO), C-reactive protein (CRP) levels. Results The expression of APRIL and BAFF mRNA in AIH group (0.65 ± 0.39,0.76 ± 0.35) was significantly higher than that in hepatitis B group (0.33 ± 0.26,0.37 ± 0.24) and normal control group (0.30 ± 0.25,0.23 ± 0.20 ). The expression of APRIL and BAFF mRNA showed a moderate positive correlation (r = 0.418, P <0.05) in type AI AI, hepatitis B and normal controls. APRIL, BAFF mRNA expression and disease activity index (ALT, AST, GLO, IgG, CRP), liver inflammation and fibrosis score was not significantly correlated (P> 0.05). Conclusions The expression of APRIL and BAFF mRNA in type Ⅰ AI H patients was significantly increased, and the level of APRIL and BAFF mRNA correlated with the prognosis of patients.
