患者女性,46岁,郊区农民。午餐后觉上腹部胀满,恶心,坐卧不安,夜间呕吐3次胃内容物,症状逐渐加重,当地医院拟诊:食物中毒。体检:痛苦面容,大汗淋漓,呼吸浅促,上腹部膨隆,胃轮廓可见,无蠕动波,有压痛,叩诊呈过度回响,并有振水音。B超检查:上腹部显示一巨大的无回声区,其内有大小不等的强光点、光团浮动。无回声区的右上方,隐约显示-3.5×3.5cm的不均质的实质性回声。急诊内镜检查:胃腔膨大,无蠕动,有大量污秽液体。
Female patient, 46 years old, rural farmer. Suffering from abdominal distention after lunch, nausea, restlessness, vomiting three times at night stomach contents, the symptoms gradually increased, the local hospital to be diagnosed: food poisoning. Physical examination: painful face, sweating, shallow breathing, bulging upper abdomen, stomach contour visible, no peristaltic waves, tenderness, percussion was over-reverberation, and vibration sound. B-ultrasound: the abdomen shows a huge echo-free area, which has a range of sizes of light spots, light group floating. The upper right of the anechoic zone shows a subtle, substantive echo of -3.5 × 3.5 cm. Emergency endoscopy: swollen stomach, no peristalsis, a large number of foul liquid.
